CONSENT WITH YOUR CARE AND TREATMENT
You will have an initial assessment as the first point of contact with the service. We will agree a care and treatment plan which will be co-produced and with your consent we will include your support network. It is helpful for people who are caring for you to be included as this will assist and support your care and treatment.
Your support network know you well and may have views on your care and treatment. They will also be able to provide an overview of your difficulties/challenges and the impact. Their involvement in your care and treatment could help enable you to progress with your interventions and for them to gather further insights and understanding of your presenting difficulties.
We will routinely review your care and treatment upon your request or if there is a need. This will ensure we are providing the care and treatment you require and provide us with the opportunity to review and formulate your care and treatment. This will support your recovery and your changing needs and circumstances.
We encourage communication with people in your support network and we will discuss your care and treatment with transparency, so we are clear of the services being delivered. We would be delighted to answer any queries/questions in relation to your care and treatment and will accept communications through a variety of methods.
We act in your best interests and will advocate for you when necessary. This may require for us to share information we hold about you due to concerns or as required. We will always apply rigorous consideration and exploration before releasing any information, and where appropriate we will inform and consult with you prior to doing so. In certain situation where there could be risk/welfare concerns and depending on the levels of risk we may disclose information to other agencies. This is because we have a statutory and professional duty of safeguarding responsibilities and accountability to you. We endeavour to work with your network and other partner agencies to deliver the best care and treatment.
Definitions:
CONSENT – in healthcare this is called ‘informed consent’, and this is where you agree to receiving the proposed care and treatment. This can be withdrawn upon your request, and we will routinely review when circumstances change to aid your recovery and meet your needs.
CONFIDENTIALITY – this is where we only tell others about your care and treatment who need to know. We will not show/release information we hold, including documentation, working notes, letters, computer records (not an exhaustive list) with anybody other than need to know. We will inform and consult with you unless it relates to your risk/welfare and is deemed necessary. This will be discussed in our multi-disciplinary team.
